We are a direct primary care organization in Avondale Arizona. At Daisy Family Health Care, we offer integrative, holistic, and functional alternative healthcare services that provide options to help you feel good from the inside out.

The DPC membership is only $1200 per year, and includes yearly labs (cbc, cmp, G6PD, lipid panel, and A1C), all your visits, discount on additional labs, and 25% on our integrative services, supplements and more.

The DPC membership can be paid monthly, at $100 per month. Pay for the year up front and save 10%.

There is no contract, cancel at anytime, with a 30 day written notice.

We will draw your labs on your first wellness visit, we have an in house pharmacy, and carry a variety of supplements in house, making Daisy a one stop shop for our members. Once you have completed your labs, and did your follow up visit, you can be cleared for our other services.
